The Social Worker will provide case management services to children, adults and their families on the Autism Spectrum. Take responsibility for providing case management and/or transition planning support to put patients. Conduct intake appointments and provide active follow-up. Develop and maintain a network of community referrals and resources for treatment and educational, residential and vocational programs in the Westchester/NYC area. Create programming to support families at all different stages of transition planning. Lead parent/caregiver support groups and provide short-term family therapy regarding transitions or focused issues. 


This position will work Monday - Fridays from 8:30am - 5:00pm at the NewYork-Presbyterian Hospital/Westchester Division.


Preferred Criteria

  • Experience in case management 
  • Experience in finding services and building a network of connections for a vulnerable population of patients


Required Criteria

  • Fieldwork or previous experience in a psychiatric setting
  • Competency in individual, family and group treatment modalities and treatment techniques appropriate to an inpatient treatment setting
  • Knowledge base of community and discharge planning resources and processes
  • Master’s Degree in Social Work (MSW)
  • NY Licensed Masters Social Worker (LMSW)
  • Limited Permit (must supply exam date prior to start of employment)
  • Basic Life Support Certification
