With a variety of hospital based and community based settings and several sub-specialties, including Psychiatry, Internal Medicine, and Pediatrics, there are several paths for success—and countless opportunities to impact the communities we serve.  The primary care sites are all designated as NCQA Patient Centered Medical Homes.  The ACN provides high-quality care to a diverse group of patients, regardless of socioeconomic status. Our leadership takes great pride in being able to stay involved in every stage of care, from the development of best practices to the coordination of multidisciplinary services. The ACN is also participating in several strategically important hospital initiatives including care management, readmission reductions, and Medicaid redesign programs.Step into this pivotal role, where you will you will help develop and oversee a broad portfolio of clinical quality improvement projects across the Ambulatory Care Network. This position will work to align multiple initiatives – including the Medicare ACO, DSRIP, PCMH, and Meaningful Use – to ensure high likelihood of success at the individual practice level. You will organize, manage and facilitate quality initiatives and staff, as well as information technology and analytic resources to meet the goals set by ACN clinical leadership and yourself. Work across the Ambulatory Care Network to lead a team of Project Managers to drive key changes and initiatives at a local level with your guidance.


Preferred Criteria


  • Master's Degree with 5 years of experience
  • Familiarity with quality and performance improvement methods
  • Lean Six Sigma or PMP certification, or commensurate QI experience

Required Criteria

  • 3 - 5 years of management experience
  • Experience in healthcare and/or community-based setting
